Formé Urgent Care and Wellness Center Opens in White Plains
Formé Rehabilitation, Inc., a physical and occupational rehabilitation service provider for 15 years, expanded its services last week with the opening of Formé Urgent Care and Wellness Center, located at 7-11 South Broadway, White Plains.
Calling itself a “boutique” medical facility, Formé provides both urgent care and specialty services in the areas of endocrinology, pain management, podiatry, vascular, pulmonology, urology, dermatology, and cosmetic and anti-aging services — including vitamin infusion medicine. There will also be a Certified Social Worker and a registered dietitian on staff. Additionally, Formé has an onsite outpatient surgical suite for minor medical procedures including, but not limited to, gastroenterological, ENT, Orthopedic, podiatric, vascular and cosmetic procedures involving, oral, local and moderate intravenous sedation.
The center is open seven days a week from 7 a.m. to 10 p.m., 365 days a year. No appointments are necessary for urgent care visits. The City Center parking garage is located directly behind the center. Formé accepts most insurance and all no-fault and worker’s compensation cases.
